· Monitor different TB register and TPT register, documentations (Contact Tracing, TPT Card, CI form, X-ray/ MT register) with coordination of DOT center.
· Preparation of vehicle plan for TPT support and provide it to District Supervisor, DOT center, UH&FPO/MODC as per need from Community level.
· Organize TPT camp with collaboration of local government and USAID's ACTB Staff.
· Planning and collecting information (Index Patient) form DOTS corner.
· Preparing weekly and Quarterly TPT report.
· Monitoring different TB and TPT register and documents.
· Secondary follow up of TPT enrolled person.
· Maintaining liaison with District Supervisor.
· Supervise Community Volunteer to carry out TPT activities related to catchment areas.

· Maintain GRN, HAK’s Received notes, Challan, Master roll, and other documents for distribution.
· Ensure the distribution of 8100 HAKs.
· Arranged and settled different types of local vendors such as local transport, decorator, Warehouse, etc.
· Set up the venue before the distribution day.
· Ensure different types of facilities on the day of distribution.
· Purchase different types of materials that are required for the distribution.
· Monitor different activities on the day of distribution at the distribution center.
· Ensure HAKs in the early morning before the distribution at the distribution point.
· Ensure the proper payment to the different parties at the end of every event.
· Ensure the tasks as per emergency instruction basis.
· Ensure the daily report at the end of every event.
· Maintain communication with the Project Coordinator as per instant requirements through Phone and e-mail.
· Prepare and submit all the bills related to the distribution.
· Collection of clearance certificates after the end of the event from the UNO.
· Submission of the event report at the end of the project.
· Identify TB presumptive, organize sample collection center and ensure transportation of the collected sample to the nearest laboratories.
· Ensure DOT of diagnosed TB patient.
· Encourage HIV testing among TB patient and high-risk population maintaining confidentiality Massage dissemination and build community awareness related to TB, Malaria, HIV and Covid-19 (use of mask and encouraging for taking vaccine).
· Refer to the nearest testing facility by identifying Covid-19 suspects and keep proper records in the register.
· Ensure testing of Malaria suspects through RDT, treatment and follow up in the malaria endemic areas.
· Identify close contact of TB patient (contact investigation), referral and ensure TPT among eligible.
· Regular follow up and coordination of Community Mobilizer, Shasthya Shebika and community health worker.
· Conduct refreshers training and distribute drugs for TB Patients.
· Maintain regular coordination, communication, strengthen referral linkage with the different health care providers (formal and informal) and other stakeholders including government.
· Keep records and reporting according to programmatic formats and registers.
· Prepare monthly work plan and implementing programmatic interventions accordingly.
· Ensure the safety of team members from any harm, abuse, neglect, harassment and exploitation to achieve the program’s goals on safeguarding implementation. Act as a key source of support, guidance and expertise on safeguarding for establishing a safe working environment.

· Conduct PRA Session, Short List the Household numbers and Fill-up Questionnaire to find the Actual Beneficiaries under rigorous selection process.
· Select Enterprise according to Actual Situation after Regional Office Verification, Train-up Selected Members according to their Selection, Purchase and Distribute Supporting Assets, Purchase assets from nearby local market and distribute these among the members.
· Provide Group and Home Visit four times in a month for Every Member and aware them about Health, Social, Family, Education, Disasters and Others Instant Issues. Monitoring their Assets, Ensure Vaccination, Proper Foods and Medicines for Assets and ensuring sell after Maturity. Maintain and update Ledger, Member Books and All Others Official Documents According to Project Needs.
· Distribute All Financial and Others Non-Financial Support among Members. Provide Medicine to the Members suffering from diseases. Provide Humanitarian Support during Covid-19. Record, Analyze and Shorting data through software with specific organizational framework.
· Identify physically disable communities, widows, divorces among the participants and provide them support with the collaboration of Social Welfare Department.
· Support to ensure all components of the project is monitored and the results are evaluated in line with the indicators.
· Support to project Office to prepare monthly, quarterly, half annual, annual and project completion report of the project.
· Ensuring ‘Safeguarding’ Everywhere Under the Project.
